The items equipped share the same kind of bonus? Because the bonuses don't stack. Only the highest is applied, the others are suppressed. You can see all the suppressed bonuses scrolling down in your character sheet.

If you can't detect the secret on level 9, you probably need more points in mechanics. Or stand more close to the button. There is 2 special doors on level 13. One is in the middle of the level, in plain sight. It's a puzzle door. You will learn something in chapter 2 that will allow you to solve the puzzle. The other door is a secret door and, as Varana and stage pointed out, you can open that one only if you absorbed the old watcher's soul.

I'm playing at 1280x1024 and that is not the case with my monitor. I only have a minor issue with the panels, for half a second they are in the wrong position before they settle in the middle of the screen. But the size is right, and I can see them almost entirely. Maybe relevant, but your screenshot is 1152x864. That is a 4:3 resolution, while 1280x1024 is 5:4.

Talking to "the man who waits" gives you 4800xp. You don't need to speak to the Warden. That chest is part of a different quest:

Any fan fix that could mess with that?

Deleting the content of Steam "downloading" folder sometimes help. Check the antivirus quarantine too.
