Damage reduction is based on weapon damage. For instance 1,000 armour rating with power armour reduces ~50% of 1,000 mini nuke damage. And I'm pretty sure certain attacks like poison/acid and deathclaw attacks are always full damage even in power armour.

I noticed stealth attacks mostly still seem to work under [Caution]. Probably because popping a head with a suppressed weapon still alerts everyone.
