Never paid attention to those as I have my eyes on the enemies' tooltips and the combat log most of the time - but I'll make an educated guess and say that they mean:

 

\!/ - you don't have enough penetration against the armor of that fellow

\?/ - the armor rating against the type of damage you are dealing is yet unknown - you have to hit that guy first in order to find out.

Thank you very much, they were driving me crazy.

I interpreted them differently. I thought they had something to do with the enemy's awareness of you. I still don't know what the blue shield with an eye icon is.

I have seen the ? icon when enemy becomes aware of you. Not sure what it means in combat

The "?" is when they notice a stealthed character and move to investigate (yellow circle turns to red)

 

The "!" is when the red circle completed and stealth ends.

 

At least, that's what is supposed to happen. I have on more than a few occasions noticed these when battle begins but no one was stealthed. Especially when using abilities, I think it's a bug triggered by the "noise" stat of some abilities, particularly at the start of a battle.
