Are there specific triggers or is it time based?

Specific triggers along the way BUT they must be with you at the right time and right place.

In answer to your question in the thread title, all companion quests can be completed before jumping the pit and none continues after defeating the main antagonist.

When they grey out in Journal are they done or are there hidden extras?

Like Eder or Aloth??

Most, if not all of them, have dialog regarding events that occur towards the end of the game. What you say in those conversations actually determines the endings for some of them.

Aloth definitely has more dialogues after his quest is complete and pretty sure they tie into his ending.

Is there any list to these triggers at end game or near it?

Can I console force them to occur?

trying to micro manage my party is getting so aggravating in order to get as much of the story elements as possible.
