Well, you can pick older Bioware games, Baldur's Gate series. I don't know what you mean exactly when you say "similar" though.

Jade Empire

Jade Empire is probably your best bet for a recent-good vs. evil-title. Different combat system though.

Bloodlines is good, as are the old IE games.

 

However, I concur with whoever said Jade Empire. It's the closest to KOTOR, IMO.

I'm waiting for Jade Empire to be release for the pc :) and I finished bloodlines for over a year ago :/

As a few others have mentioned, try the Baldur's Gate series. Although there aren't two distinctly different ways to play the game depending on if you are good or evil, they are still some of the best RPGs ever created.

 

If you're still hungry for more, play the Fallout series and Planescape: Torment.
