Have a similar issue. There are 2 phantoms that did not die after encounters in which they were summoned by Kana. They won't die or dissapear. I've tried to kill them out of combat, they'll appear dead but if I get in combat again they are back alive. There's no way to dismiss them and they appear in every screen I'm at. They also attack sometimes before I even do. I didn't realize how big of a problem it was until recently. I don't have savegames I can revert to, unless I restart from the beginning.
