In the Man Who Waits Quest I talked to the Head Warden, then to Moedred who told me about the agitated patients. When I go back to the Head Warden he does not offer to give me a key to the North Ward, so I am now stuck.

So what about if you talk to Riply, find the notes, go up and talk to the Head Warden, allowed into the patient area, and Azo isn't in the lab (or anywhere in the sanitarium)?

Cellar, last office on the right. Pillage the chest that's there in the corner. You get the patient records, which is everything you need to convince the head warden to hand over the key.

...but not the key to the North Ward. That you have to get from Azo, and if he doesn't exist downstairs, you're out of luck.
