It's possible to create a saved game where the player character is maimed and that leads to an instant loss when loaded.

In a dungeon maim the player character then save the game. When you reload the game the player character (and any other maimed companions) will fall over with the death animation usually they will get back up but some times the save gets into state where after they fall over it triggers the "Game Ended" or 'You are Dead' screen. This seems to mainly happen if you accidentally save a couple of times quickly in a row, but the result is that save is no longer usable.
