Take a look into the file "swkotor2.ini" which can be found in the Kotor2 main directory. You can insert the resolution settings there if they are missing, and try again without using the Configuration Utility.

Does your computer meet the minimum requirements to play the game? When you click on the Scan Hardware button, what does it show you for Video?

Do you have anything listed like in my screen cap?

Make sure your graphics card is capable of running the game. (My suggestion)

If you are running a screen refresh rate of more than 85 the game will crash on startup and as you have already seen the configuration utility doesn't work properly. Lower your refresh rate to 85 and if you are using some form of refresh rate overide software make sure that the resolution you are going to run the game in is set to 85. From what I can gather KOTOR 2 does not support a refresh rate of greater than 85.
