For the other two points... just keep playing. They'll happen.

Lootra is in the apartments in the "docks" part of the city. His wife is stuck in the Refugee area and they won't let her out, nor will they let Lootra in. I don't know for sure, but if you go talk to him then go back to talk to his wife (who's standing right next to the "leader" of the refugees) you can probably still get credit for the quest as long as you clear out all of the Exchange thugs and I think you also might need to run around a little bit near the exit to the Refugee quarter.
