I played two games of quest mode and haven't seen the bonus from the patch notes. Am I missing something?

Correct.  Play Quest Mode now, and after it disappears in a coming patch, you'll get some goodies to remember it by.

Yes, that seems to be what the devs indicated.  

 

If have played Quest Mode between that last announcement and the coming patch, you will get one set of rewards based on the stats of your highest-level character.  If you have multiple lvl 40s, you will not get multiple piles of loot.
