Not sure what's causing it, but some fights, enemies have this blank (white square) debuff (up to 4 of it) named Sagani Attack (noticed mostly in od nua and sun in shadow). Sometimes it has no name and is just a white square (that happened in the WM2 bog area vs cultists).

I loaded up your save and went to said areas, but I never saw the white boxes. Are you on 1008?

Yes and cache is verified

And I didn't get them every battle just some battles where everyone has fighting. I couldn't reproduce it using Sagani only.

 

Since most places are cleared of monsters in the save, it may be worth trying spawning a bunch of tough enemies and let the party loose to see it?

Yeah, I tried to see what buffs could be applied based on your party make up and no white boxes. I also just let sagani just sit in the middle of combat and get beat down all day, but no issues occurred.
