After completing the quest 'Skulls for Mother' I was was to receive the Father's Teeth neck slot item.

 

I don't appear to have been given the reward; I have looked in my stash and all my characters including the one I sent on the adventure. I have tried reloading the game and still nothing.

Welcome to the forums and thanks for joining us! Did you check your Stronghold Stash? It will be in the lower right room in the Great Hall. Thats where Stronghold items and rewards will be placed.

It was in the stronghold stash! I derped so hard.

 

I could have sworn I checked there, but I guess I didn't.
