Question about druid spirit animal form deactivating

Enjoying a druid as my main character, but don't understand why his spirit animal form (bear) keeps deactivating amidst combat. Maybe I missed it, but I didn't see anything anywhere about a duration (and the duration seems to vary) or damage, etc. causing deactivation.  Any explanations?
