Question about Companion Approval

I've noticed that in Deadfire, there are multiple instances where companions' approval or disapproval pops up as a result of random npc's saying something, or other - let's say weird - circumstances, as in my sig (where the alternative was to leave the people in question to their troubles). It's most obvious with Aloth as his reactions have a tendency to be negative, but others do it as well. For instance, talking to the people on the upper floor of the Inn in Queen's Berth, or paying for their services triggers a number of positive reactions from companions. Aloth's buddy will get positives from a number of characters, and the actress from Aedyr will even trigger a positive comment from Aloth, but no approval, while Xoti and Serafen will give the worldy tag, which does actually increase your approval with them (I went to + 1 with Xoti and Sera). Tekehu gave me a positive pious reaction for chanting NEMNOK (?!)

Is this intentional? If it is, is it good?

It also makes me wonder if the reactions we get to companions saying something are sometimes reactions to the player, and not one another, or both. If that were the case, getting positive/negative approval would be easier/more difficult depending simply upon who's in your party.

I'll get the ones we have here bugged so we can start combing through them.

So does that mean that these seemingly random gains/losses are legit bugs? While the positive ones are less annoying (lol), it does seem exceedingly bizarre / beyond one's control, and I'd rather gain/lose approval normally.

It very much seems so. I got positive/negative reactions from companions by initiating conversation with NPCs or entering location. Whenever tags got messed up, or something else is going on - no one knows. Considering that Caleb asked for an attatched save, that means that this might be a more complex issue than simply conversation trees being tagged.
