The achievement seems to ping on how many sidekicks you've recruited, rather than picking up all of the ones currently available.

 

For example, Rekke is still floating about somewhere yet to be recruited and I've just picked up Mirke, Konstanten, Fassima and Ydwin (maybe), so the formual is currently ALL SIDEKICKS IN GAME - 1 = ACHIEVEMENT.

I guess they haven't had time to implement the change due to the DLC release.

 

Somewhat related, if you sacrificed Eder or Aloth in PoE 1 and import/decide on that story, you can't recruit them in PoE 2 and therefore cannot get the all companion achievement.
