I am in the desert near Poko Kohara.

I was fighting the worms hidden in the sand. I killed all of them, but one follows me and attacks me but I cannot select or attack it.

I already have this listed in my thread. It can also happen with the "Grubs" in the ruins under The Gullet.

 

From my testing, it seems to occur if you kill the grub/worm while it's popping out of the ground. Still partially burrowed, but can be targeted. The game recognizes it as being "dead", but it will continue attacking since it was locked out of its death animation due to the "unburrow" animation superseding any other animation.

 

You can just run far enough away from it to end the combat and then walk back. This usually clears it up. If not, change zones and come back. When you come back there's even a nice little loot pile for you, if it actually dropped something. So generous!
