No bug. through story you will learn something that allows you to progress. It is sort of a way to force you to come back at a higher level because what you will fight within. I think you're talking about the door to finish level 13 and the OP is talking about the very top right corner of the map where there is a treasure room. I am quite sure this is the result of the rope vision you get after killing Maerwald. If you take the knowledge, this is the knowledge that you get. I made that choice and was able to open the door. I am, however, unable to open the door to progress and I guess you saved me some searching.

I was doing the quest for the Vailian ambassador and I had to fight the group once they found out I wasn't the real buyer. That's the first point my character started doing 2000+ damage per hit. Even though the weapon stats are normal, look at my stats on the left (Defense are weird too): I'm not sure if this is related, but it's also something buggy that I noticed:

I'm still only on Act II, but I've talked to Sagani a few times and she keeps bringing up my search for Thaos after we talk about her search for Persoq. The only problem is, I can't for the life of me ever remember reading anything about a Thaos other than when she says it. Is this just a case of Obsidian getting the timing wrong and she's saying something earlier than she should?

This worked for me: once the game is attached to your Steam profile (i.e. you can see click the link to see your achievements), just start a new game. When you get to the caravan campsite after character creation, it should pop.

Once your game is linked to your Steam profile (i.e. your achievements show up), just start a new game. My achievement popped after creating a character as you enter the caravan campsite.
