I've been wondering about it, too. I started playing PoE soon as I was able and let the DLC finish downloading. I received the backer item, but the achievement doesn't show as obtained.

Same issue here and the same Hearth Harvest Axe like Devard. Seems to be the only item with the issue, but will need to check the rest of the stash tabs before confirming.

Same bug hits me. Any items in inventory and quick slots disappear when dismissing a companion. Always have to remember to strip them of inventory and quick slot before dismissing, which does get a touch annoying in the long run.
