The western Old City to Old City Ruins exit does not place you in the proper location, rendering the upper left pool area in the Old City Ruins inaccessible.

Thank you for your post. It is intended for the top left portion of the map in the Old City Ruins to be inaccessible. The area transition places your party in the appropriate location by design when coming from the Old City to the Old City Ruins.
