Easiest issue to reproduce is:

1. Switch to Bear form in combat
2. Switch to Boar form before Bear form expires
3. Finish combat
4. You're now a permanent Boar

Other issues, harder to reproduce:

• Switching between forms doesn't work and destroys all equipped gear
• Switching between forms works momentarily, but new form cancels almost immediately, though it is still available to switch to again

All on MacOS through Steam.
