I encountered Elyrium at the Catacombs of Wrath. After successfully evading the first Sinspawn she summoned, it again gave me the "encounter or evade?" choice for Elyrium, and when I chose encounter, spawned an additional Sinspawn. And again, when I lost to the next Sinspawn. And so on...
To be fair, I haven't actually defeated any of the Sinspawns. But I don't think this is intended behavior.
