The graphic difference between Permadeath on and off is very subtle and would be hard for anyone with color vision deficiencies to understand at a glance. You might consider something more obvious like a white/black toggle.

Just had permadeath on and didn't realize it. Super pissed now. This is a big issue; I don't even understand why that button can be turned on mid-game, or why it doesn't have s confirmation dialogue. Unknowingly triggering permadeath is a seriously painful mistake to make as a player.

Apologies for the inconvenience. Permadeath can only be turned on during character selection when you are forming a new party. Permadeath will be highlighted if enabled and a notification will pop up. I'm not quite sure how you were able to enable permadeath during an already progressed game. There is no such option to do so.
