There is a loot location that contains a cloak of Fortitude in the second area (Requires a gem to reveal the location), Loot All results in it appearing on none of my characters and not in my inventory. The video demonstrates 2 interactions, one of them results in successful looting and the other demonstration results in the item disappearing from the universe.

Please check out the Beta patch and see if this issue is still occurring. I have already answered this in your original post so its duplication was not necessary.

It does seem like this has been fixed in the current beta, I'm unable to reproduce the problem.
