I noticed this after getting down to the very bottom of level 15 of the Endless Paths, after you have to scale down the broken stairs - during the scripted interaction, Aloth supposedly fell and hurt himself - but the "Twisted ankle" injury appeared afterward on my PC, not Aloth. I think this has happened in other areas but I didn't really notice it, this was the first time I thought it was weird.

 

I don't have a save from before going down, but here's the save showing the injury on my PC, and my system specs.
