Eternity Backer: Obsidian Order of Eternity survey

I'm having trouble what the Obsidian Order of Eternity name survey is for, is it the name of my character or is it a phrase/title for my character that would go with the name?

if possible can someone add examples

It's the phrase/title, for me it's 'mascot of the obsidian order' as shown above my avatar

it can be either a title or a name these were going to go into the game credits as far as im aware but I think these should have been filled in quite a while ago, or it may just be the forum version from to get your title.
