I spoke with Inez at the Ministry of Accuracy, she left and went back to Milversmeet.  I went back to the town hall in Fairfield to find no Inez, and I no longer have an option to talk to Milversmeet, only Taur.  A similar bug happened when I first started the game and I never received my TTD.  I had to restart the game.  I'm like 10 hours in now, not doing that again.

Also, when using a controller sometimes F5 won't quicksave until I click with the mouse.

I have a similar Bug, I did explore a lot of the island, I found Inez and after I while I entered in the Town hall wher Milversmeet is talking to Aunty on the big screen. After the "cinematic" , there is a text as if spoken by Inez, although she is not there nor Milbersmeet speaks. He remains blocked, I can not speak with him nor find Inez

I'm having a similar issue (still), I chatted with Inez at the ministry (because why would I do the main quest before I've done all the side quests?). Later I dropped in at town hall to chat with Milversmeet, Inez told me to meet her outside but she's just been standing there telling me to not work with the guy. I figured this was a bug and that it would reset if I left and came back, unfortunately I came back way later and Inez is still stuck there repeating the same "don't work with this idiot" bark and won't move off the spot.
