Got a rare Potion of Perserverence with the Liquid trait from a treasure chest. Used it on my drunken master monk with the "Bring me another!" power, but never got prompted to try and recharge it. After the scenario the card was indeed banished from my deck. I would like the card back because it seems to only be available in treasures chests. 

I was in Rimeskull on Legendary and the forced movement from closing locations seemed to be interrupting the regular order of prompts, maybe that's why it happened?
