As I was on the ship I talked to Max and in mid dialogue one of my crew member climber an invisible ladder in the background and clipped through the ceiling  next thing I know Pav is labeled as “dead” even though I’m not on supernovae and her quest Don’t Bite The Sun is botched thankfully I had a save before hand but has anyone else had crew members just “die” on the ship I was in the space laboratory.

I am playing on Supernova and Pav died on the Unreliable for me as well. Unfortunately I didnt have any prior saves to bring her back. Failed companion quest but still had option to have her in my party but as soon as you leave the ship it says she left party and died.

I'm having nearly the same problem. The quest "Don’t Bite The Sun" is botched while Parvati is alive and well. I can talk to her and take her along. 

Cannot complete the quest. :(
