My characters are dying everytime they are knocked out in a fight. That started happening when I picked up pet Grog, but I don't have him equipped anymore.

Thank you for sending me that save file!  This is a super strange occurrence as I have not seen this outside of Berath's Challenge or having three wounds.  I was absolutely able to reproduce this with your save file, so I have sent it off to Programming to investigate the issue so they can get a fix out as soon as possible.  I hope this hasn't been hindering your playthrough too much.  If you discover anything else about this, please let me know and I will add that information to the bug report.

I just loaded an older save and this time when I unequipped pet Grog the permadeath mechanic stopped as it should. I'll let you know if I encounter this again, but for now that's all I know.
