One thing to double check, did you ever play on the file on a lower difficulty level? It seems you have to go through the whole game on Path of the Damned Difficulty to get the achievement. if you can drop the difficulty down without getting a warning that it'll invalidate the path of the damned Achievement, than it's likely the file was flagged as having been on a lower difficulty at some point. It's something to double check, even if you never dropped the difficultly, it could of some how ended up being set that way.

I've had a hard time trying to figure out what causes the quest to break, as I've had saves where it worked, then after playing 2 or so minutes, the quest fails to appear, even with the exact same equipment on both saves. I've tried engaging in combat with some other blue (fine) quality enemy before Thallia, and all other sorts of things to try and find out what prevents it from triggering correctly, but I haven't had much luck in trying to find an exact trigger just yet. Whatever the case, it's not limited to just already having fine quality gear, or even high enough ranking gear. At the very least I think it shouldn't be counted toward the completed all sidequest achievement, as at the very least, it's unpredictable as to if you can even get that quest based on unknown factors (beyond just already having good gear)

At the end of the Heart of Valor quest if you refuse the give Keipo the Heart, you will always receive the Heart of the Leviathan (trinket), even if you chose to give the heart to the Tira Nui, Hajime. If you chose to keep the heart, you will get the trinket right away, and then again as the quest completes, resulting in two copies of the Heart of the Leviathan Trinket. Dialogue chain I tested: With Keipo: 1: We need to talk. Now. 1: Come clean. Chiko told me you want to end your life. 2: What if your age is just catching up to you? 2: I'm sorry. I can't give you the heart. 1: I respect your choice. But I can't be a part of it. With Chiko and Hajime: 1: of Course not. I couldn't put you through that. With turning in the heart: 2: [Give him the heart.] Here. Whatever happened, it's better off staying with you. (quest item heart of the Leviathan removed) 1: Uh... About that. He's not just sick. It's the dreamscourge. 1: He wanted to die before he turned. He was trying to keep you safe. 2: If I knew, You'd be the first one I'd tell. (receive stormdancer set) (quest ends) (receive Heart of the Leviathan Trinket) With keeping the heart: 3: Let me hold onto it. I'll take it far away and keep it safe. (heart of the Leviathan Trinket added) 1: Uh... About that. He's not just sick. It's the dreamscourge. 1: He wanted to die before he turned. He was trying to keep you safe. 2: If I knew, You'd be the first one I'd tell. (receive stormdancer set) (quest ends) (receive Second Heart of the Leviathan Trinket) Note that when choosing to keep the heart, the quest item version of the heart does not get removed.
