Explorer Achievment

tell me if this was discussed before, but i only found this topic: http://forums.obsidian.net/topic/78192-explorer-achievement/?hl=explorer

Its a little bit old and people didn't know to much back then.

 

So, can anybody tell me how this achievment works? Do all the places have to be explored in ONE game? Does "explore" mean enter the map?  Or enter specific parts of each map (the ones that give exp?)

On my first playthrough for example I couldn't acess the room on od nua level 13 in the northeast corner despite high mechanics. (I suppose because I didn't consume Mearwald's soul at the beginning).

 

Has anybody gotten that achievment? During the middle of the game (would mean the achievment is tracked across games) or at the end?
