To reproduce:

 

1. Have a female PC.

2. Go talk to Sîdha and Rîhenwn in Elm's Reach.

3. Be referred to as "he."  In voiced dialogue.  Sîdha manages to avoid mentioning gender either way, which makes it doubly confusing to me how this happened unless two people wrote (and recorded) their dialogue separately.
