Inventory items disappeared and rollback

I was in stronghold, and used the stronghold button to edit my party. Later on when edit my party again after playing for a while, I found that all the items in the inventory of a removed party member went missing and one of the member got a roll back of some kind, because I switched her weapon before removing and I got the old weapon on her when I got her back.

 

Too late for me to try and repeat the bug, maybe someone has encounter it as well.
