I am on a Windows 7 system and simply can not get past the third stage of Randy's abortion, (suctioning)

 

I have double checked updates, I tried on casual, nothing I hit on keyboard seems to do anything.  Some posts say there a comments when too switch, I have none of that.  How on god's earth is this task supposed to be accomplished?

 

I am totally frustrated at this point, if I can't complete the game, what should I do
