"Crash Points"

Okay, I've been playing this game for several months now, had some trouble getting it to work on Vista but no real issues after figuring that out. Thing is, lately there have been points in the plot at which my game just crashes, unconditionally, no explanation. First noticed it in a game that kept crashing right after I arrived on Nar Shadaa; I went around and did the whole rest of the game with no problems but could not reach Nar Shadaa without a crash. I gave up on that and started a new game, but this one crashes whenever I try to leave the Polar Academy. My newest one crashes as soon as I hit Citadel Station. Nothing seems to work and I'm on my last nerve here. My system specs are as follows:
