Can't Throw Grenades Sometimes (PS5)

There are times when I am unable to throw grenades/distraction devices.  Sometimes I can restore the ability to throw if I do some/any/all of the following:
1) Crouch and Stand up again.
2) Press/hold the Triangle button to open the weapon wheel and select a different grenade.
3) Open the inventory window and move a different grenade type from inventory into a grenade slot.
But, most of the time I cannot restore the ability to throw.  I have tried pres/hold R1 in order to see the grenade trajectory - no trajectory arc appears.  I have tried a quick press/release of R1 in order to just blindly throw the grenade and it does not work.

I have posted this on r/theouterworlds and r/theouterworlds2  and I have one Redditor is reporting the same problem and they are playing on XBox.  (they didn't specify exactly which platform)
