How to receive the Jal Shey Mentor armor

I followed a strategy guide on how to receive the Jal Shey Mentor armor from Fassa on Nar Shadda. Your character is supposed to be at level 40 or better. After leveling up to 40 and finishing the tasks Fassa wants you to finish you're supposed to receive the armor. When I try it Fassa doesn't give me anything. What am I doing wrong?
