At the end of the Heart of Valor quest if you refuse the give Keipo the Heart, you will always receive the Heart of the Leviathan (trinket), even if you chose to give the heart to the Tira Nui, Hajime. If you chose to keep the heart, you will get the trinket right away, and then again as the quest completes, resulting in two copies of the Heart of the Leviathan Trinket.

 

Dialogue chain I tested:

With Keipo:

1: We need to talk. Now.

1: Come clean. Chiko told me you want to end your life.

2: What if your age is just catching up to you?

2: I'm sorry. I can't give you the heart.

1: I respect your choice. But I can't be a part of it.

 

With Chiko and Hajime:

1: of Course not. I couldn't put you through that.

 

With turning in the heart:

2: [Give him the heart.] Here. Whatever happened, it's better off staying with you.

(quest item heart of the Leviathan removed)

1: Uh... About that. He's not just sick. It's the dreamscourge.

1: He wanted to die before he turned. He was trying to keep you safe.

2: If I knew, You'd be the first one I'd tell.

(receive stormdancer set)

(quest ends)

(receive Heart of the Leviathan Trinket)

With keeping the heart:

3: Let me hold onto it. I'll take it far away and keep it safe.

(heart of the Leviathan Trinket added)

2: If I knew, You'd be the first one I'd tell.

Note that when choosing to keep the heart, the quest item version of the heart does not get removed.
