Q about hot bar and inventory.

I have a few questions about managing inventory and repairing tools, weapons etc.

1. How do you move an item from the hot bar to the inventory and vice versa?

I press A on an item to move it between the 2, but either nothing happens or the item just disappears. 

2. I can't repair some items.

Sometimes when I try to press RT to bring up the sub menu of an item, the repair option is greyed out.  

I make sure to unequip the item thinking that's why its hreyed out, but that doesn't help.

What's weird, is that sometimes I can repair items. It seems totally random.

3. When an item is in my hot bar and it has lets say,  20% durability,  the same item in the inventory has 100% durability.  

Why does it show different durabilities in each menu?

Can't you also press Y to repair?
