Pebblet spear disappears from hand when attacking

As earlier with the pebblet axe, the pebblet spear now disappears from the hand when you hit an enemy once (the appear isn't breaking or anything, you have to re-equip it after each hit)
 

Not sure what triggered the or the same issue with the axe
