The companion quest drinking sapphire wine is now listed as botched in my journal because "pavarti was killed". I did not think that pavarti dying would be an issue as i am not playing on supernova difficulty. the journal says it happened in between introducing pavarti to junlei and speaking to pavarti aboard the unreliable. However i was able to speak to pavarti on the unreliable and get the correct dialogue about junlei and  drinking at the lost hope and such, but when i go to the lost hope no further dialog can be triggered.

I have the same issue. I can take her as part of my squad, but she is no longer in the ship and the quest is listed as botched.
