In my other run there is no problem regarding this.

 

There are two major differences in my two runs (if it is of any help):

  1. In this run I have already completed "He waits in fire" quest. In the other run that quest is still on hold.
  2. I have used "iroll20s" in this run for Devil of Caroc armor while the other run still give me achievements.

 

Found the reason. Its not a bug.

Configure browser push notifications
Chrome (Android)
  1. Tap the lock icon next to the address bar.
  2. Tap Permissions → Notifications.
  3. Adjust your preference.
Chrome (Desktop)
  1. Click the padlock icon in the address bar.
  2. Select Site settings.
  3. Find Notifications and adjust your preference.
