This is with a new game on the current patch.  I did a respec on my lvl 6 Inquisitor (main character) and suddenly I have 2 level 1 fighter powers.  Nothing seemed to removed, but these clearly don't belong on my character.  I've attached a screen shot of the active effects.  The knock down is also showing up on the quick bar.  Let me know if you need the save game (I'll keep a copy before reverting back to pre-respec).

Thanks for the update.  Saw some posts from earlier releases, but wasn't sure if it was still a known issue or not.
