Its not a fix but when I completed her second companion quest both her upgrades became available. So if you have this bug, I suggest you prioritize her second companion quest if you prefer using inez in your party.

Same issue. Hopefully obsidian can add her upgrade for those of us that missed it. Shouldn't be to difficult since its an item. Please fix this obsidian.
