I have the exact same issue, I have collected everything including molars, scabs, mutations, cards, wendell, stuff and I am missing ONE thing in places. Hedge Lab Ascent I ran pass this location god knows how many times, and it still haven't triggered. This is definitely a bug. Btw I am playing on steam in multiplayer.
