    I'm not sure it is a bug or just a part of the game. I directly killed Kaoto in the map Halls of Presence because I triggered the alarm in the Halls of Silence. However, after killing Kaoto and reaching the relinquary, he is still alive and guard there. You can see Kaoto has been killed according to my journal but just stands before me.
