Same problem on PC, picked up Scales of the Oathbinder before getting the totem base and hours later I realized its no longer in my inventory, nor has it been placed on the totem. Its missing from the location you find it and my quest log says I've found them all. Pretty annoying bug and hope there is some kind of patch to fix this soon.
