I am also experiencing something similar, fortunately for me combat ends and I don't really mind having an animat or phantom running around with the party. Once they're killed they go away. Did you try targeting your own summoned creature and killing it?

Description: When hitting an enemy with very low endurance the combat log does not report the Deaths Usher effect. Steps to Reproduce the Issue: Create a Death Godlike and tickle a mob to Death. Note that there is no report of Death's Usher happening when you hover your mouse over the combat logs. I had taken the Apprentice Sneak Attack Talent, which was being shown when applicable.
