I had finished Far From Home and had given Serel 6000 cp for the amulet so that she now hangs around the fountain in Brackenbury. Later when I entered the district I'm attacked by assassins by the fountain. The first thing they did though was to concentrate on murdering Serel before turning to me. I managed to save her from getting killed by loading and killing them quickly.

After talking to the Head Warden Ethelmoer I was given the task of talking to the Animancers and asking about anything unusual. I had previously talked to everyone in the sanitarium and exhausted all conversation options, so they were greyed out. I had finished Aloth's quest there, talked with Nans about the scepter and sold it to her and also talked with Moedred about Helig of Thein and given him the key to the chest before talking to the Head Warden. Now I could ask Nans about the leaden key but when I started the conversation with Moedred there was no new option. Only after I had asked him again about some previously asked points did the option appear.
